USSPT Photo & Video Release Form
By submitting this form, I hereby grant USSPT, LLC, its owners, employees, agents, and assigns (collectively “USSPT”) the absolute and irrevocable right and permission to:
- Photograph and/or record me (including my name, image, likeness, voice, and any statements or comments I make) while at the USSPT facility or during any USSPT-related activity, class, event, or interview.
- Use, re-use, publish, re-publish, edit, modify, and publicly display such photographs, video footage, and audio recordings (in whole or in part) in any and all media now known or hereafter developed, including but not limited to the USSPT website, social media accounts (Instagram, Facebook, TikTok, YouTube, etc.), email campaigns, printed materials, advertisements, and promotional videos.
- Use my name, city, and any biographical information I provide in connection with the above materials.
I understand and agree that:
- All photographs, videos, and recordings will be the sole property of USSPT.
- I will receive no payment or other compensation for the use of my image, likeness, or statements.
- I waive any right to inspect or approve the finished product or the use to which it may be applied.
- This release is binding on me, my heirs, and assigns.
I am 18 years of age or older and have the full legal capacity to give this consent.

Manufacturing defects are defined as any issues related to the production and manufacturing of the item. These defects do not include damage caused by improper use, unpacking, or handling of the item once it's in the possession of the buyer.
