USSPT Photo & Video Release Form

By submitting this form, I hereby grant USSPT, LLC, its owners, employees, agents, and assigns (collectively “USSPT”) the absolute and irrevocable right and permission to:

  • Photograph and/or record me (including my name, image, likeness, voice, and any statements or comments I make) while at the USSPT facility or during any USSPT-related activity, class, event, or interview.
  • Use, re-use, publish, re-publish, edit, modify, and publicly display such photographs, video footage, and audio recordings (in whole or in part) in any and all media now known or hereafter developed, including but not limited to the USSPT website, social media accounts (Instagram, Facebook, TikTok, YouTube, etc.), email campaigns, printed materials, advertisements, and promotional videos.
  • Use my name, city, and any biographical information I provide in connection with the above materials.

I understand and agree that:

  • All photographs, videos, and recordings will be the sole property of USSPT.
  • I will receive no payment or other compensation for the use of my image, likeness, or statements.
  • I waive any right to inspect or approve the finished product or the use to which it may be applied.
  • This release is binding on me, my heirs, and assigns.

I am 18 years of age or older and have the full legal capacity to give this consent.

Welcome to www.us-spt.com (the “Website”). These Terms & Conditions (“Terms”) govern your access to and use of the Website, membership at USSPT (the “Facility”), and all related services provided by USSPT, LLC, a Texas limited liability company (“USSPT,” “we,” “us,” or “our”).

By accessing the Website, signing up for membership, entering the Facility, or using any of our services, you agree to be bound by these Terms. If you do not agree, you may not use the Website or Facility.

1. Membership & Facility Use

  • Membership is personal, non-transferable, and subject to the rules posted in the Facility and any membership agreement.
  • You must be at least 18 years old to purchase a membership or sign these Terms. Minors under 18 may only use the Facility with a parent/legal guardian’s signed waiver and supervision as required.
  • We reserve the right to refuse entry or terminate/suspend any membership at any time for violation of these Terms, Facility rules, or conduct that disrupts operations or endangers others.

2. SMS / Text Message Marketing & Notifications

By providing your mobile phone number at sign-up, on the Website, or in the Facility, you expressly consent to receive recurring SMS/text messages from USSPT, including:

  • Promotional offers, discounts, and marketing messages
  • Event announcements and schedule changes
  • Membership renewal reminders and payment alerts
  • Facility updates (closures, new equipment, etc.)

Message frequency varies. Message & data rates may apply. You can opt out at any time by replying STOP to any message or emailing info@usspt.com. Carriers are not liable for delayed or undelivered messages. Supported carriers include major U.S. carriers; international and some smaller carriers may not be supported.

3. Payment & Billing

  • All fees are non-refundable except as expressly stated in writing.
  • We may change membership rates with 30 days’ notice.
  • Late payments may result in suspension of access until the balance is paid.

4. Assumption of Risk & Waiver of Liability

You acknowledge that physical exercise involves risk of injury. You voluntarily assume all risk of injury, illness, or damage and agree to release, waive, and discharge USSPT, its owners, employees, and agents from any liability arising from use of the Facility or participation in any activity.

5. Accessibility Statement

USSPT is committed to making our Website accessible to all individuals, including those with disabilities. We strive to follow the Web Content Accessibility Guidelines (WCAG) 2.1 Level AA. 6. Photo / Video Release

By entering the Facility, you consent to be photographed or recorded. USSPT may use your image, likeness, and voice for marketing and promotional purposes without compensation (see full Photo & Video Release on the Privacy Policy page).

7. Intellectual Property

All content on the Website and all USSPT branding are owned by or licensed to USSPT and protected by copyright, trademark, and other laws.

8. Privacy

Your personal information is governed by our Privacy Policy, available on the Website.

9. Governing Law

These Terms are governed by the laws of the State of Texas. Any dispute shall be resolved exclusively in the state or federal courts located in Denton County, Texas.

10. Changes to Terms

We may update these Terms at any time. The updated version will be posted on the Website with a revised “Last Updated” date. Continued use of the Website or Facility after changes constitutes acceptance of the new Terms.
